Hi everyone, I am needing some more defined clarification on the PRE-Bereavement Risk Assessment that is to be completed within the 5-day compliance window with a new patient admit. I am new to my role and trying to understand everything. 1) What have you found is the best productive way to get these completed? 2) What do you do if you have a bereavement contact that is marked yes, but you cannot get into contact, and they do not return calls? - what is the best way to document that, and do you change the contact to NO bereavement? 3) Are we able to email the pre-risk assessment and have completed along with admission paperwork that we send to family members who are medical POA to sign if they cannot be present at admit? 4) Who exactly is able to do the PRE-Risk Assessments? I know the MSW, CMRN, and Chaplain can for sure- but can LPN's, CNA's and or community Liaisons help with them? 5)How do you handle a note etc. should a family member decline to do the risk assessment and declines bereavement services?
Sorry I know those are a lot of questions, but I am just really struggling and keep getting conflicting answers. Any and all help would be greatly appreciated.
