If both the HOPE Admission file and Discharge file were submitted and accepted by iQIES, and the sequence issue is listed as a 'warning' on the Final Validation Report, the records are valid and would not require resubmission. If any of the HOPE files were not submitted, or were submitted but rejected (fatal error), then follow up would be required.

I recently had a HOPE Discharge record come back with a warning -924d (inconsistent record sequence). This was a discharge and the iQIES sent back this message as it did not recognize the admission file already submitted on the patient. After review, I found that internally my hospice team has corrected the patient's name and so the two files were not matching. The discharge record was accepted. I created an inactivation request on the admission file - which was submitted and accepted. I corrected the admission file and then that corrected file was admitted and accepted. My question is do I need to do anything with the discharge file (resubmit so that it shows correct sequence of events with the now corrected admission file, or leave it alone as already accepted but has the warning message on it)??
