If the patient is new to your hospice a HOPE-Admission should be completed and the HUVs scheduled based on the admission date as would be the case for all patients admitted on or after October 1, 2025. The HOPE tool does not follow the patient's care across hospices. It is designed to capture data specific to the hospice in which the patient is enrolled.

Hello! We had a patient transfer from another hospice agency on day 18 of the election of their first hospice benefite period. Since it was a transfer, the continue in that benefit period. We are confused on how to schedule the HOPE visits (specifically HUV1 and 2). Should they be scheduled based on the ELECTION DAY (18 days before our agency admitted them) or on the ADMISSION DATE (day 18 of the first benefit period). Reading the Hope Guidence manual, it feels like these are used interchangable (see example below from page 14). Any clarification would be appreciated. Thanks so much!
