Hospice

I'm curious about the HUV and potential subsequent SFVs when someone transfers from home to an inpatient setting.  We currently create a new care plan when someone transfers and I'm wondering how we will track those visits.  I also wonder about those requirements for patients who travel outside of our service area and we contract another hospice - do we tell them the required timelines and they conduct the appropriate visits???
