    On Monday, the Centers for Medicare & Medicaid Services released its 2025 list of Measures Under Consideration (MUC). Among 24 new or updated measures, just one would apply to the Home Health Quality Reporting Program (QRP): Advance Care Planning. There are no measures under consideration for the Hospice QRP on the December 2025 MUC List. Advance Care Planning would measure the percentage of patients 18 and older who have an advance care planning document or documentation of an advance care planning discussion resulting in a documented decision in the electronic health record by the time of discharge. The MUC list is part of the agency's process for identifying, reviewing and publicly listing quality and efficiency measures for potential adoption in Medicare programs. While inclusion on the MUC does not guarantee adoption, measures that are endorsed by the consensus-based entity often appear in the spring and summer payment rules.
