    Looking for best practice - do you complete a discharge summary or simply a decedent note?  And if you complete a discharge summary, what prompted you to do so? What do you include in your summary vs note?

    Our hospice has a "Hospice Death Notification Note" template. This gets sent to all hospice staff and the hospice attending.
