Hospice

We are trying to create consistent policies regarding certifications and have encountered a couple of scenarios that we are unsure how to handle. 

The first one is: If the patient chooses a community physician as their attending physician, can we get a verbal confirmation from a covering physician in their practice if they are unavailable through the third day? Similarly, if the patient chooses our medical director as attending, can we get a verbal confirmation from another one of our hospice physicians if the medical director is unavailable through the third day?

The second question is, if the certifying hospice physician is different from the physician reported in the Attending Physician field on our first billing claim to Medicare, must we have a signed CTI from that physician?
