These may not be the correct "communities" to post this question in so please pass it along to the right one(s).
I have a question regarding care plan verbiage. When you are creating a care plan for your pt/family/caregiver, do you ever use "throughout the hospice stay" as your measurable? Or do you limit it to each benefit period/one benefit period at a time?
SN example: if a pt is incontinent of bowel and bladder, do you write a care plan for maintained skin integrity for each benefit period or do you write one care plan for maintained skin integrity "throughout the hospice stay"?
MSW example: if a family member/caregiver seems to be having a hard time with his/her loved one being admitted to hospice, do you write a care plan encouraging/assessing healthy coping for each benefit period or "throughout the hospice stay"?
In my opinion (as a QAPI Coordinator and not a clinician) continually creating the same care plan over and over is an indication we are not educating properly because the care plan closes at the end of one benefit period and starts back on the next. If the care plan is "throughout the hospice stay", that indicates ongoing education for the entirety of the hospice stay.
Please correct me if I am wrong or misguided in my thinking!!
