    We are seeking to understand ways programs are following up with surviing family members or survivors after the loss of a loved one.  Are you sending a memorial gift / acknowledgement, a meal, or only following up through a bereavement counselor and resources. We would appreciate any thoughts.

    After the death of a loved one, we send the assigned bereaved family member a sympathy card signed by the hospice staff and an ornament with their name added to it. At the year anniversary, we send a memorial mailing. This is all on top of our bereavement phone calls we do every few months if needed.

    We send a single rose to the person listed as primary bereaved, along with a sympathy card, and have received comments from families that this was meaningful to them.

    We are a small, not for profit hospice and do all our follow up via our bereavement program schedule.  We also send a white rose to the visitation / funeral if we have the information on the services to do that.
