We performed a re-cert visit within the 5-day window for a patient. The next visit was to be performed in the new cert period, but the patient went to the ED with S/S of a UTI and was hospitalized. We are trying to determine the proper sequence for documentation, and if it's out there, I'm not looking in the correct place.
Do you complete a missed visit note and a Transfer OASIS? Do you perform a DC OASIS?
