Can anyone share the life cycle process of a SOC at your agency from receiving the referral to creating the poc? For example, the process at our agency is: (1) intake receives the referral, chooses billable primary diagnosis (2) SOC clinician opens patient to home health, completes OASIS documentation and careplan (3) Clinical managers perform OASIS/chart review and email clinicians for corrections (4) Coding reviews and codes referral, reviews care plan, communication notes, and orders. Emails clinicians for any additional corrections found (5) Coding closes the OASIS and creates the poc (6) Nurse reviews poc and signs.
We have internal coding and use MatrixCare as our EMR. I'm interested in anyone with insight, both for internal and external coding as well as other EMR's, but particularly MatrixCare.
