    CMS released the Preview Reports and Star Rating Preview Reports for the October 2025 refresh of Home Health Quality Reporting Program (HH QRP) data on July 9. One agency has reported highly unusual results in their data. We are interested in learning whether other home health agencies are experiencing similarly unexpected or potentially erroneous results.

    Hi Mary,

    Yes, our report also contained erroneous information in the OASIS outcomes, reporting our results as single digit percentages.  I have emailed CMS via iQIES email address about the error but have not heard back yet.

    Hi Mary, our Care Compare Preview Report was significantly erroneous - scores in the single digits. Stars preview was seemingly correct, though.

    Hello Mary,

    Our outcomes percentages were 0% the first time I checked the report despite having numerators/denominators. I rechecked the next day and it was updated but the calculations do not appear correct. They are off by 1-2% on some measures. I am emailing them today.

    I pulled our reports last Friday and the math is off by 3-4% on several measures, but not all of them, which is curious.
