    Q: Is a new Face to Face required if a patient is being discharged from Medicare and admitted under Medicaid. 

    Scenario: Client is on services under Medicaid; we have a face to face for med management.

    She goes into the hospital and is discharge to home with therapy and wound cares, with a new face to face done for hip pain and wound cares.   

    We discharge her Medicaid episode and complete a SOC under Medicare. 

    Client is discharging from the Medicare episode, and will have a new SOC under Medicaid for medication management. It has been more than 90 days since the original Medicaid face to face for med management. They only gap in care is 7 days per client request due to travel.
