  • 1.  Face to Face by acute hospital physician

    Our home health agency currently receives a face-to-face attestation statement along with the home health referral orders from the attending and/or discharging physician in the hospital.  Their f2f attestation statement currently asks that the hospitalist name the community physician; however, this may be blank or inaccurate once home health is set up. Are other agencies requiring the hospital physician name the following physician?  How do you handle this scenario? Thank you.

    We send a Physicians Attestation form that requires the PCP to sign that they have received it.  It just has the referring physician, date F2F was done, and the PCP on it with a signature line for the PCP.  It works.  The PCP is wrong so much we found this way better and more time efficient for our needs.  It's faxed or emailed to PCP whichever way they prefer, faxed or emailed back to us.  It's not very high tech but it works.
