If the myalgia is due to the original injury, I wouldn't even change the primary diagnosis. You can add that myalgia dx (if the provider has diagnosed the patient with that diagnosis), but I would just keep your primary as the dislocation of joint. Your original F2F runs the entire course of the episode of care, NOT the cert period. Although it shouldn't happen often, you can have a patient open through multiple cert periods under the original F2F. The attached checklist tool is from CGS website (our MAC). Hope this helps.

Subject: F2F & Dx change at recert
Is a new F2F needed? And does the provider who completes that F2F then also have to sign the 485?
Patient admitted under Medicare from an inpatient facility for dislocation of a joint (shoulder), due to myofascial issues, OT recertified the patient and changed the diagnosis to myalgia, which is due to the original injury.
