My agency has had issues with field clinicians not identifying the last ordered visit, failing to provide the NOMNOC, as well as losing track of patients when the last ordered visit was not identified. I am interested to see if other agencies are using any functionality within thier documentation system to prompt the clinicians that ordered visits are nearly completed.
I do realize that ongoing discharge planning is truly the answer here, which would negate the need for the system to prompt clinicians. Any suggestions related to teaching and enforcing discharge planning would be appreciated as well.
